Looking for COHC related form in Word/Excell

Trying to re-vamp a COHC log, individual client record sheet, or good
database program to use. I'm open to suggestions, as this is a new
business, and we are starting a program at ground level. Is there a site
that has ready made medical forms in Word or Excel?

if you are looking for a good database w/ security you might want to go to

it has both a desktop and a pda database.
